Dermal fillers and biostimulators are injectable products that can diminish the appearance of age-related volume loss, fine lines, wrinkles, and folds on the face, neck, and body. Dermal fillers, largely composed of synthetic hyaluronic acid (HA), are used to fill areas where volume is deficient, whereas biostimulatory fillers prompt the body to create new collagen over time for a filling and plumping effect.

Both dermal fillers and biostimulators are considered minimally invasive treatments for restoring a youthful appearance. They are injected into the skin instead of the deep layers of fat or muscle, giving way to a long-lasting and natural improvement in the appearance. While both are excellent options for restoring volume to the treated areas, there are notable differences between dermal fillers and biostimulators.

Dermal fillers rely on hyaluronic acid (HA) and are not as long-lasting as biostimulator fillers. However, they attract water to the treated area for a volumizing effect.
Biostimulator fillers directly impact the production of elastin and collagen levels within the skin, giving way to added fullness over time and lasting longer than dermal fillers. These fillers act as tiny scaffolds, adding volume to the treatment areas and helping reduce the appearance of lines and wrinkles. Some biostimulators may also give a slight lifting effect due to increased collagen production.

How Do Dermal Fillers and Biostimulators Work?

Dermal fillers and biostimulators have different modes of action. While the end goal of both treatments is a more youthful appearance, how they achieve this differs.

Dermal fillers fill in wrinkles, lines, and folds, making them less visible. Certain types of dermal fillers can also be used in specific features to add volume and definition or shape the contours of the face. For example, when the appropriate filler is injected, dermal fillers can enhance the lips and cheeks.

If you’re looking for a full face makeover but don’t want to opt for  surgery, an instant facelift may be right for you. Our expert injectors specialize in non-surgical facelifts, which utilize hyaluronic acid fillers to provide full facial rejuvenation without the risks or downtime associated with traditional facelifts. 

The most popular hyaluronic acid dermal fillers include Restylane® and JUVÉDERM®, specifically JUVÉDERM® VOLUMA® XC and JUVÉDERM® ULTRA PLUS XC. These shorter-acting dermal fillers have different consistencies, properties, and thicknesses, making each appropriate for specific areas of use. For example, JUVÉDERM® VOLUMA® XC can enhance the cheeks, whereas Restylane® is a firmer, more flexible filler that works well on areas like the lips. Unlike biostimulators, hyaluronic acid dermal fillers are fully reversible with a few quick injections of hyaluronidase, which dissolves hyaluronic acid if you are unhappy with the results.

Biostimulators

Biostimulatory fillers, including Radiesse® and Sculptra®, create volume by stimulating collagen production. Calcium hydroxylapatite (Radiesse®) and poly-L-lactic acid (Sculptra®) are injected deep into the skin (into the dermis) to stimulate natural volume via collagen production, producing effects that typically last upwards of at least two years.

Radiesse® volumizes the face and hands and corrects facial lines and wrinkles, such as nasolabial folds. Made of calcium-based microspheres suspended in a water-based gel, Radiesse cosmetic filler is injected into the skin using a small needle to immediately soften folds and wrinkles and add volume to the face and hands. Radiesse® provides long-lasting, natural results by restoring collagen and elasticity to give volume and lift to the skin, diminishing the signs of aging.

Sculptra® is also an injectable biostimulator that gradually replaces lost collagen. It relies on poly-L-lactic acid (PLLA) to restore the skin’s structure and smooth wrinkles. Sculptra® is administered as a series of injection treatments to achieve long-lasting results.

The results of biostimulators aren’t as immediate as those of dermal fillers. Instead, they gradually appear over the coming months as the body creates new collagen. While the results can’t be reversed as they can be with hyaluronic acid dermal fillers, biostimulators can provide longer-lasting improvement.

The injection process for dermal fillers and biostimulators is similar. First, we recommend a consultation to ensure you qualify for one or both injectable treatments. Then, once the appropriate injectable has been chosen for you, the treatment area is cleansed and sterilized before the injectable product is injected into the desired location to improve its appearance. The area is numbed, and a thin needle is used to inject small amounts of filler under the skin. With dermal fillers, more of a superficial injection occurs; biostimulators are injected deeper into the dermis. The injections take 15 minutes to an hour, depending on the treated area
Post-treatment, you may be instructed to gently massage the treated area, whether dermal fillers or biostimulators are used, to ensure a more even distribution of the product. With both injectables, minor swelling, redness, or bruising is to be expected, which should subside within a few days. You’ll want to be gentle with your skin, avoid excessive sun exposure, and keep the injection sites free of harsh skincare activities while it heals.
With dermal fillers, the results are immediate, with the exception of some mild swelling at first. However, to reach optimal results, both treatment options may require multiple injection sessions and maintenance treatments.

This option provides longer-lasting results, making biostimulators a good solution for those who don't want to repeat their injections often.

Both dermal fillers and biostimulators are ideal solutions for those not ready or willing to undergo surgical treatments and wish to enhance their appearance with next to no downtime and few risks.

Hyaluronic acid fillers, like Restylane® and JUVÉDERM®, are very safe since hyaluronic acid is a naturally occurring sugar molecule in your body. If there are any issues with these fillers, we can safely inject an enzyme called hyaluronidase into the treated area to dissolve it instantaneously. Side effects are very rare.

Treatment with any dermal filler can cause mild swelling, which usually subsides in a few days. Swelling may last longer with thicker products, like Juvederm Voluma, or if more than one syringe is used. Swelling is usually more noticeable under the eyes and around the mouth.

To minimize swelling, ice the area for 24 to 48 hours following the procedure. You can also apply arnica ointment, or take oral arnica tablets to help reduce bruising and swelling.

The longevity of your results depends on the type of filler used, the treatment area, and individual factors like metabolism and skin thickness. Generally, dermal fillers can provide results that last from six to 18 months, whereas biostimulators can last one to two years or longer.

Whether you choose to get dermal or biostimulator fillers, it's important to avoid blood thinners of all kinds for at least a week before your appointment to minimize bruising. You'll also want to keep your body well-hydrated by drinking plenty of water and avoiding alcohol before your appointment. Make sure you tell your injector about all current medical conditions, allergies, and medications you are taking to prevent any complications.
