Inside The M Factor: Menopause and Hormone Questions Answered

Serenity Aesthetics & Wellness recently partnered with Biote to host The M Factor, a free documentary screening followed by a live Q&A on menopause, hormone replacement therapy, healthy aging, and wellness. The evening drew a room full of women ready to ask the questions they’ve been holding onto for years, some of them since their first hot flash.

Our functional health providers, Jodi Choplin (MSN, NP-BC) and Lexi Yoo (FNP-BC/CPNP, IFMCP), led the discussion and answered questions candidly. This recap walks through the questions asked that night, with responses polished for clarity but true to what our clients heard live.

If you missed it, this is your seat at the table. And if you’d like to continue the conversation, our functional health team offers hormone replacement therapy consultations at both our San Francisco and Burlingame locations.

Key Takeaways

  • Hormone replacement therapy is about long term protection for your brain, heart, and bones, not only symptom relief.
  • The difference between bioidentical hormones and synthetic ones is significant, and the delivery method changes both safety and results.
  • Comprehensive labs and true personalization are the foundation of safe, effective HRT care.

Why Should You Even Consider Hormone Replacement Therapy?

Menopause is natural, but so is the drop in protection that comes with it. Estrogen is deeply protective of the brain, which helps explain why three to four times as many women as men are living in nursing homes today with Alzheimer’s dementia. Cardiovascular disease is the number one cause of death in women, and research suggests hormone replacement therapy can lower that risk by at least 20%.

We’ve had these hormones in our bodies since we were in our mother’s womb. The problems start when they leave. Replacing them thoughtfully helps protect the next few decades of your life, not just the next few months of symptoms.

Is There an Age When I Have to Stop HRT?

Many women have been told they must stop hormone replacement therapy at 65, 70, or some other arbitrary cutoff. That guidance is worth questioning.

The later decades of life are often the most delicate. They’re also when the brain, heart, and bones need protection the most. Stopping HRT, then, when your body has finally been supported for years, doesn’t always make sense.

The decision is fully individualized. It’s less about symptoms at that age and more about whether you want the last twenty years of your life to be active, independent, and clear-headed. Your provider should be walking you through that decision with your full health picture in mind, not applying a blanket rule.

Jodi shared the case of a 68-year-old client who came to our clinic already past the standard HRT window. She started pellet therapy with estradiol and testosterone.

Within a year, her DEXA scan improved by two standard deviations, a jump so significant her outside doctor assumed she’d been placed on Prolia. Her answer was simple: “I went to that hormone lady, and I got some pellets.” That kind of protection is what personalized BHRT can offer when it’s started with the whole body in mind.

What Does Progesterone Actually Do?

Progesterone is one of the most misunderstood hormones in menopause care. Most women associate it with periods and pregnancy, but it plays a much broader role.

When progesterone is low, women often feel it as poor sleep, daytime anxiety, mood swings, and irritability. It’s a deeply calming hormone that supports the nervous system. Replacing it can shift how the day feels almost immediately, often within the first few weeks.

Progesterone is also protective of the bones, brain, and heart. At Serenity Aesthetics & Wellness, oral progesterone is the most common form because it works well for sleep and mood support, though other options exist depending on the individual.

Can You Explain How Estrogen Is Delivered?

Estrogen can be replaced through pills, patches, creams, injections, or pellets. At Serenity, pellet therapy is one of the most popular options because it closely mirrors how the body naturally produces hormones.

Pellets are about the size of a grain of salt. They’re placed in the subcutaneous fatty tissue of the upper hip during a fifteen-minute in-office procedure that’s largely painless. The pellet then releases hormone slowly and steadily over three to four months.

Other delivery methods work well too. But many clients report the best results and fewest side effects with pellets because a steady dose feels closer to what the body is used to receiving on its own.

Why Are My Joints Suddenly Hurting?

Joint pain in your forties and fifties often gets blamed on aging or overuse, but hormones deserve a closer look.

Estrogen helps lubricate the joints. When it drops, women frequently develop frozen shoulder, tennis elbow, or unexplained joint pain, sometimes with no injury at all. Many end up in physical therapy or even surgery without anyone ever checking their hormones.

Ruling that connection out early can change the entire treatment path. Sometimes the answer isn’t more physical therapy. It’s addressing what’s happening underneath.

Isn’t Testosterone Just for Libido?

Testosterone is often reduced to libido, but that’s only one piece of the puzzle.

Testosterone is the hormone behind your energy and get-up-and-go. Many women describe losing interest in workouts they used to love, and testosterone is often part of that story. It also supports mental clarity and motivation.

Beyond how you feel day to day, testosterone is protective of bones and the cardiovascular system, and emerging studies suggest it may be protective against breast cancer. That’s why our functional health team considers testosterone as thoughtfully as estrogen and progesterone during any HRT consultation in San Francisco or Burlingame.

My Doctor Said I Can’t Have Progesterone. Why?

The first question to ask back is: What kind of progesterone were they talking about?

Synthetic progestins like Provera or the mini pill are not the same as bioidentical progesterone. When synthetic forms pass through the liver, they can raise clotting factors, elevate an inflammatory marker called high-sensitivity C-reactive protein, and increase triglycerides. That’s where many of the historic concerns about breast cancer and blood clots trace back to.

Bioidentical progesterone, on the other hand, is chemically identical to what your body already makes and offers systemic protection: brain, heart, and bone health. For an overview of hormone therapy, The Menopause Society is a well-vetted resource.

Does the Way I Take Testosterone Really Matter?

Yes, and this is one of the most misunderstood parts of hormone therapy.

When a hormone passes through the mouth into the gut, it goes through the liver first, which changes how the body processes it. Oral forms of testosterone carry more risk because of the first pass. Testosterone injections, pellets, and topical forms bypass the liver, and studies support that difference.

That’s why a woman whose doctor told her “no” to testosterone might actually be a strong candidate for a different delivery method. It’s not the hormone itself that carries all the risk. It’s often the route.

What Labs Do You Run For HRT and Why?

Many women are told “your labs look fine” while feeling exhausted, foggy, or anxious. The disconnect usually comes down to what’s actually being tested and against which ranges.

Standard lab panels flag disease. They’re not designed to optimize wellness. At Serenity, our functional health team uses stricter optimal ranges and runs a much wider panel, so we can help you feel your best rather than just avoid a diagnosis.

Here’s what our comprehensive panel typically includes:

  • Full CBC and CMP for liver, kidneys, blood counts, and electrolytes
  • Inflammatory markers like HS CRP
  • Insulin resistance markers, including hemoglobin A1C
  • Nutrients like vitamin B and vitamin D
  • Full thyroid panel including T4, T3, and antibodies for autoimmune thyroid disease
  • Full sex hormones, including estrogen, progesterone, testosterone, free testosterone, and sex hormone-binding globulin

How Much Do the Labs Cost?

Insurance rarely covers a comprehensive hormone panel, and that’s a big reason so many women never get one. Serenity negotiated a discounted contract rate directly with our lab company to make it accessible.

You pay us, we pay the lab, and you never deal with a bill. Clients can use HSA or FSA funds and request an itemized receipt to apply toward a deductible if they’d like. For members, the full lab panel is included as part of the Serenity Membership, at no additional cost.

How Often Will I Need to Get My Lab Work Checked?

After starting testosterone, we recheck levels around six weeks later to confirm dosing. From there, most clients settle into visits every three to four months, and never longer than six months apart. During perimenopause, when hormones are still fluctuating, the first year usually involves more frequent visits until things stabilize. Many women say they didn’t even remember what normal felt like until we brought them back to it.

Frequently Asked Questions About HRT

Do I need to have severe symptoms to start hormone replacement therapy?

Not at all. Many women who start HRT at Serenity have mild or vague symptoms, or none yet, and want to focus on long-term protection for the brain, heart, and bones. Others are simply tired of not feeling like themselves. Whether you’re deep in perimenopause or just noticing the earliest shifts, a consultation with our functional health team can help you decide if now is the right time for you.

Is bioidentical hormone replacement therapy safe for women with a family history of breast cancer?

This depends heavily on your personal history, current health, and which hormones are being considered. Bioidentical progesterone in particular does not carry the same risk profile as synthetic progestins, and emerging research on bioidentical testosterone is encouraging. A thorough consultation, comprehensive labs, and careful ongoing monitoring are essential. Our providers review each case individually and never recommend a plan without fully understanding your risk factors and goals first.

Can I start HRT in San Francisco or Burlingame if I’m still getting periods?

Yes. Many of our clients begin hormone support during perimenopause, when periods are still happening, but hormones are already fluctuating. Symptoms like poor sleep, anxiety, mood swings, and low energy often appear years before menopause is official. Starting BHRT during this window can smooth the transition significantly. Your provider will tailor the plan based on where you are in your cycle and what your labs and symptoms show.

How soon will I feel a difference after starting BHRT?

Most clients notice meaningful changes within the first two to six weeks, though the full picture continues to unfold over three to six months. Sleep and mood often improve first, followed by energy, mental clarity, and libido. Because we recheck labs around six weeks in, we can fine-tune your dose based on both how you feel and what your bloodwork shows. That’s why side effects are rare in our practice.

What makes Serenity’s approach to menopause care different?

The short answer is time, testing, and personalization. Our consultations are longer than a standard appointment, our lab panels go well beyond what most primary care offices run, and our optimal ranges are calibrated for how you feel, not just for avoiding disease. We also offer bioidentical hormones exclusively and monitor closely so your plan evolves with your body across perimenopause, menopause, and the decades that follow.
